Palliative Care
Our professionally trained sisters provide specialised community based nursing care to patients in the familiar and comfortable setting of their own homes. Each patient is treated according to an individually created treatment plan best suited to provide medical support, education and care. Should there be additional needs that require attention, the community sister will connect the patient or family to other members of the palliative care team.
The Palliative Team consists of four palliative trained nursing sisters with years of experience in palliative nursing, supported by a medical administrator and social workers. Our Nursing Manager oversees the department.
Psychosocial Care and Bereavement Support
The adjustment to living with a life-threatening diagnosis can be challenging, and at times lonely. Our social worker and spiritual team are available to support the patient, and loved ones, in coming to acceptance of the diagnosis and to optimising every moment of life.
Finding meaning in suffering and loss will assist the patient and family to live optimistically despite the diagnosis, and experiencing much needed appropriate support. Centurion Hospice support extends to the patient’s and family’s needs in a holistic approach that addresses physical, social and spiritual needs.
Clinical Services
Palliative Care services are typically provided through regular physician and nursing visits.
Hospice Care services are more inclusive than Palliative Care services. Hospice Care includes physician services, nursing services, social worker, spiritual care, bereavement care and volunteers. In some cases physical, occupational, speech and dietary therapy services, as well as other counselling services deemed necessary as part of the Hospice Holistic Care Plan to manage terminal symptoms and provide support for the individual and their family.
