Palliative Care

Our professionally trained sisters provide specialised community based nursing care to patients in the familiar and comfortable setting of their own homes.  Each patient is treated according to an individually created treatment plan best suited to provide medical support, education and care.  Should there be additional needs that require attention, the community sister will connect the patient or family to other members of the palliative care team.

The Palliative Team consists of four palliative trained nursing sisters with years of experience in palliative nursing, supported by a medical administrator and social workers. Our Nursing Manager oversees the department.

 Community Based Palliative Care 

  • Patient assessment by a trained sister in hospital or at home.
  •  
  • Skilled, compassionate care from the sisters making intermittent visits.
  •  
  • Pain and other physical symptoms managed through the appropriate use of medication and teaching.
  •  
  • Advice provided by consultant doctors with Palliative Care training and medical prescriptions where needed.  The consultant doctor does not replace the patient’s own doctor.
  •  
  • Training, advice and skills for care givers and loved ones who assist in providing patient care.
  •  
  • Emotional support and spiritual counselling for patients, family and caregivers.
  •  
  • Emergency telephonic advice after hours and weekends from the sister-on-call.
  •  
  • Referral to after hour nursing services and/or care workers.

 

In-Patient Care at a designated Care Facility

  • Admission for pain and symptom control, respite care and terminal care.
  •  
  • Patients are admitted when their care can no longer not be provided at home.
  •  
  • Patients are admitted for a period of 7-10 days, and not for long-term care.
